yes it is.

– Create a 100% discount coupon, choose a coupon name (e.g. auto-every-4th-item) and a description (e.g. Black-friday offer).
– On the usage restrictions tab set min qty of matching products to: 4
– On the Usage limits tab choose ‘Limit discount to:’ ‘Every nth item’
– On the miscellaneous tab tick ‘Auto coupon’
